For more than 28 years, our specially trained professional health care team has been providing care and support to people with advanced and progressive illness in Schuyler, Steuben, and Chemung counties. Our services are available to individuals with a life expectancy of approximately six months or less regardless of diagnosis, age, sex, race, religion or ability to pay. We're here for you and your family, and we want you to take advantage of all the services that you are entitled to.
Our Advanced Illness Team encourages you to talk to us at any stage of your illness and treatment regardless of prognosis so that you will have information you need to make informed decisions; and to assist you in accessing the help that is available from community resources.
