Life Beat: Cardiac Palliative Care Program

If you are dealing with heart failure, you and your family may be facing a unique set of needs and challenges. Your medical course frequently takes dramatic turns up and down which are hard to predict. You may face many distressing symptoms which are difficult to control, especially with little supportive care coming into your home. This can lead to frequent trips to the emergency room or admissions to the hospital, when all you want to do is stay at home.

Life Beat is here to help you and your family during this difficult time. When your heart disease has reached an advanced state and your focus moves to symptom control, you may be eligible for our specialized cardiac palliative care program, Life Beat. The Life Beat team is specially trained in the needs, including physical, emotional, social and spiritual, of cardiac patients and their families. We are aimed at helping you manage your care at home and will provide the tools you need to do so. We are available 24 hours a day to respond to changes and crises, and will come to the home if needed to deal with changes in symptoms. If a symptom cannot be controlled at home, we will arrange for and cover short-term stays at local hospitals to get everything back under control and get you back home as soon as possible. Your relationship with your primary physician remains unchanged and that provider continues to manage your care. Our team serves as the eyes and hands of that provider, working with you and your family to meet your needs.

All it takes is a phone call607-962-3100 or toll free at 1-800-734-1570 – and we will be happy to give you more information about Life Beat or arrange a time to meet with you and go over what you may need. Life Beat is covered fully by Medicare, Medicaid and most private insurances. If no insurance is present, a sliding fee scale is present. We serve all residents of Chemung, Steuben and Schuyler Counties and can bring Life Beat to residents of skilled nursing facilities and other care facilities.
