Advanced Illness Planning

Our Advanced Illness Planning team is available to meet with patients and families no matter where they are in their treatment process or what the patient’s prognosis is. We meet with the patient and family one-on-one and assess their needs. We then can get them in touch with the appropriate community services. We can also place a trained volunteer to provide support, assistance and companionship. The Advanced Illness Planning team remains available as a resource as changes occur or as questions arise. All Advanced Illness Planning services are free of charge and require no commitment or admission to STHPC. Just call us at 607-962-3100 to arrange for this service.
